The first verse is about Nate and Blair, they break up b/c he cheated with serena, get back together, and break up again.
The second is about Chuck and Blair. She watches him while she is with nate. They get together and break up b/c he is, of course, Chuck Bass. She still sticks by him after Bart dies, but then Blair and Nate get back together, but she isnt happy, b/c she misses Chuck.
The last verse is about Nate and blair and the secret person that Nate has always been in love with, that kept the relationship with blair from really working.
Then we go back and end with Chuck and Blair.

The main point of the video is that it is mainly about Blair, with the two main men in her life pulling her in two different directions. In the end, the decision is made even easier by the fact that she knows which one really loves her.
