A ventricular septal defect is an abnormal opening in the interventricular septum that results in the shunting of blood from the left ventricle to the right ventricle during systole. This shunting of blood causes a loud, harsh, pansystolic murmur that is best heard at the left lower sternal border. Palpate the carotid pulse while auscultating the heart to relate the auscultatory findings to the cardiac cycle.
