For external fixation of the tibial plateau fractures, usually, the pins are placed on the lateral side of the femur. If the pins are placed medially, lower the position of the pin on the femur
Extraarticular: reduction, fixation: plate, external fixator across the knee or below the knee
Intra-articular (tibial plateau): schatzker classification:
•Type I: pure cleavage fracture of the lateral tibial plateau.
Reduction
fixation with plate or screws.
•Type II: cleavage fracture of the lateral tibial plateau with articular surface depression. Reduction
fixation: plate or screws
•Type III: pure central depression fracture of the lateral tibial plateau with an intact osseous rim. Reduction, the inflatable bone tamp is centered under the area of depression and then inflated to reduce the depression fracture. The void that has been left by the inflatable bone tamp is replaced with bone filler.
•Type IV: split or depression fracture of the medial tibial plateau. Rule out knee dislocation. Check the neurovascular status.
Fixation: plate (BEST) or screws(less rigid than plate).
•Type V: bicondylar tibial plateau fracture with an intact metaphysis and diaphysis.
Reduction
fixation: plate (lateral, medial or both), external fixator, external fixator and plate.
•Type VI: uni-condylar or bicondylar tibial plateau fracture with metaphyseal-diaphyseal dissociation.
Reduction
fixation: plate (lateral, medial or both), external fixator, external fixator & plate.
