The Community Health Model alters health norms and raises social capital levels by providing a platform for people to communicate new cultures of health. This, in turn, reduces disease incidence and prevalence.

Jason A. Rosenfeld began his career in Ghana, West Africa as a Community Health Advisor with the US Peace Corps. He has since worked with the U.S.
Agency for International Development’s Food for Peace program in South Africa and Zimbabwe, and with community-based health education partners in Haiti and the Dominican Republic. Since 2011, Jason has served as the Assistant
Director for Global Health at the Center for Medical Humanities & Ethics.
