They’re some of the most disturbing stories in Hollywood history, and despite the precautions that are in place, on-set deaths have continued to happen during the production of films.

Some are quite well-known, like the accidental shooting death of Brandon Lee on the set of The Crow or the death of Vic Morrow and two child actors on the set of Twilight Zone: The Movie, but sadly there have been plenty of others as well.
